Moodle LMS Quiz Upload & Access Errors

I am facing two issues with my Moodle LMS:
1. When trying to upload quiz questions to the question bank, an error arises: "moodle_exception import error".
2. When a student attempts to take a quiz, they encounter the error: "Moodle_exception , this is not your attempt!".
