Monday Form on Wordpress -- 2

I need Mondays forms to collect Leads directly in my Monday.

There are 3 different forms:

1) The Homepage/blog form: https://integraliah.com/ and here for example: https://integraliah.com/hebrew-israeli-names-for-girls-300/
-> The Send button send the lead to Monday and redirect to this page: https://integraliah.com/hebrew-level/

2) The Voucher form: https://integraliah.com/learn-hebrew-for-free/
->The Send button send the lead to Monday and redirect to this page: https://integraliah.com/hebrew-level/

3) The Hebrew test form (in 2 languages english and french but they need to be totally equivalent to each other): https://integraliah.com/test-hebrew-level/ for english and I'll create a new page for test in french with the Hebrew test form in french.
->The Send button send the lead to Monday and will redirect to a Thank You page I will create when it's done.
