Mobile app development (basis questionnaire that needs to be completed)

We are in need of a very basic mobile app. It is basically a splashpage with links to our website, link to our Facebook group and a basic Q & A section. It also includes a questionnaire that needs to be filled in and the data should be sent to an email address.
