Implement User Contact Info collection

I work with Inova Devevlopment and we're an entrepreneurial enterprise that develops ideas for productivity in the app environment. We have a functioning alarm app on the iOS store and we wanted someone who could implement a way to be able to collect user contact information so we could survey our users for feedback and help direct us to areas to improve our app.
