Build an App

I am after an app, built to connect health care practitioners with their patients, allowing patients to log certain data and the practitioner be able to view it. This app will have a practitioner account, which will allow the practitioner to set up an account for their patient, select the data/content for the patient to log and as the patient logs, the practitioner will be able to view the data entry (possibly run reports etc.). For the patients side, they will log into their account, view a calendar and the data collection subjects, and enter their report to that topic. Each topic will have a custom way of entering data that the practitioner can choose, eg. text, drop down, tick box

I have may other additions that I Would like to feature, eg. notifications as a reminder to report, possible chat feature, connect practitioners Facebook, instagram, have links to their website and booking pages
