Automatic wrap measurement form generator

Please find attachments, you will find one that shows the 3 forms we send to our customers to measure for their vinyl wrap. They measure, fill it out and send it back, we thrn manually make the template in illustrator (line drawing attachment) , we make each line to the exact measurements they send within a 1/16th and line each point up , and then add the wrap to the background to ensure fit, hide the measurement layer and hit print

What i am looking for is a way to skip the making the line template , if we could have a form that we fill out a,b,c,d ect for eacu template and the lines automatically get made in illustrator, that is what we need
