APP for time registration

A simple app to register working hours.
It will have a few employee fields and a few fields where managers can sign off (in the app). currently it is done with an excel form that needs to be printed and scanned. Next the form must be send to 2 email addresses. The submitted forms must be kept in a history tab. Also option to mark them as paid.
A settings tab where some data can be entered which will be shown in the dropdown field when form is filled out.
