simple App mit Formular

ENGLISH:
Sales representatives must document the technical conditions for new companies/locations. You take photos with your smartphone, enter data into the form and send the form with the photos by e-mail from the app to the boss.
(Possibly login function. But not absolutely necessary.)
The company logo as an app icon and possibly also the logo in the app visible.
In the app is a form with several choices (drop-down menu, check boxes, etc) and text boxes for additional information. The completed form should be sent to an internal company e-mail address for data evaluation.
The data entered from the form must then be entered in the "AGP rent" program. There is no direct interface at the moment. We are open to suggestions for solutions. These must really work and be cheap!

Außendienst-Mitarbeiter müssen bei neuen Firmen/Locations die technischen Gegebenheiten dokumentieren. Sie machen Fotos mit dem Smartphone, tragen Daten in das Formular ein und senden das Formular mit den Fotos per E-Mail aus der App heraus, an den Chef.
(Eventuell Login-Funktion. Jedoch nicht zwingend notwendig.)
Das Firmenlogo als App Icon und evtl. auch das Logo in der App ersichtlich.
In der App ist ein Formular mit mehreren Auswahlmöglichkeiten (Dropdown Menu, Check Boxes, etc) und Textfelder für zusätzliche Information. Das ausgefüllte Formular soll an eine Firmeninterne E-Mail Adresse zur Datenauswertung gesendet werden.
Die eingetragenen Daten aus dem Formular müssen anschließend im Programm "AGP rent" eingetragen werden. Eine direkte Schnittstelle gibt es im Moment nicht. Wir sind offen für Lösungsvorschläge. Diese müssen wirklich funktionieren und günstig sein!
