A mobile app for AWV (Annual wellness visit)

I would like to get a mobile app (preferably created with React Native or Flutter) that asks around 18 questions (the questions are ready) and after a user answers the question He/she will get a detailed result of what he should be avoiding or start doing based on his answers. For example, if he is drinking alcohol 5 times a day and not exercising at all, the app later on, will tell him the benefits of exercising and that he must start working out. (It would be nice if this is generated with ChatGPT api but I guess ChatGPT won't allow us to write health letters)
The app is meant to be used by doctors to make their work easier, so the letter will be reviewed by them and adjusted.
