Service Ticketing (Quote) iOS App on iPad

Looking for help to build a service ticketing app on iOS (iPad for starters) to enable technicians to provide a quote to a customer before a job can start. The app should allow the technicians to capture customer's name, address, phone number, email address and signature verification, date, as well as materials needed to complete the job (quantity, description, price, total) in a table format, labor required (# of hours, cost per hour, total) in a table format, markup value (fix rate for starters), tax (fix rate for starters), and a Quote ID. Once the information is entered and submitted, it should be automatically emailed to the office and the customer's email entered in the form as an attached Excel document. This is not a social engagement app. This app is not meant to be sold on the AppStore. I have a design that I can share, as well as an Excel document format.
