Need Basic APNS Help and Implementation

Hey there. I am working on an application that needs to have Apple Push Notifications (APNS) working to notify players that things have happened. I've been working on this a long time and have yet to be able to get it working correctly with the latest updates to APNS (the last time it worked was 2012...).
I have a sample application that I wrote to test it but even that isn't working correctly. These apps are written in objective-C, PHP, and designed for the iOS platform (iPhone and iPad). I am using an Ubuntu back-end with PHP and JWT for the packaging. It seems to be working in that I have a sample APNS PHP test file that receives an HTTP 200 response (success) from the APNS service. But nothing shows up on the device.
I am pretty open to whatever can be done to make it work (except re-writing the app in Swift... :D), so if there are frameworks or changes to the backend or whatnot, I'm good. And I think everything on the app side is correct, but I think it has to be wrong given that I'm not receiving any notifications. Since APNS is SOOOO difficult to troubleshoot, I just need someone who has experience doing this successfully to help solve this problem.
