Mobile App with Dynamic form capability for Daita Entry

It is a native - Android - app.
I want a mobile app that would be used for data entry by select individuals into a NodeJS application. The mobile app will receive an uploaded text file describing list of parameters, from which it would generate dynamic forms for data entry.
The app will mostly be used in remote locations where there's no availability of internet and as such must support offline use. The app will maintain a local database of records entered into it, while syncing same records to a Nodejs server when internet service is restored. The app will support the creation and use of various dynamic forms, and data collected (from the app on the device) or other devices during sync can be edited as well.
The app will be able to provide data entry for the various types of data entities
Water Points, Schools, Latrines, Healthcare
Each of this entities has variable list of attributes that will be collected for them
