Help me with system design life cycle for developing an application for driver and customer.

I was trying to create a questionnaire for creating a food delivery application.
All i want to do is to ask questions for the customers and drivers to make my food delivery application precise.

I just need help in creating questions which can cover all concerns related food delivery application.
