Form Page UI for my Settings Registration Page

I am looking for a skilled UI designer to create a form page UI for my mobile settings registration page.
The ideal candidate should have experience in designing user-friendly interfaces for mobile platforms. The project requires a standard design with text fields and buttons. The designer should also include validation and error messages for a smooth user experience. Attention to detail and the ability to meet deadlines are essential. Validation error message relates to email address and mobile phone syntax. This should be easy and fast basically i am wanting a UI theme so there is a consistent look and feel across the 3-4 screens of the app.
Once created the layout will be provided to a third party to use as the theme to be applied to the overall application they have developed that has significant technical aspects in terms of connectivity of the app to various peripheral devices etc non of which falls within the scope of this project your role here is purely designing the UI theme. Basically a header footer for navigation buttons and page headings and then the theme style for the form elements within the form page
