I'm seeking a developer to create a smartphone app geared toward managing diabetes and hypertension. This application needs to be user-friendly with a modern and sleek design. Significantly, it should provide:

• User Profile Management: Ability to create and manage individual user profiles where users can enter personal medical details, blood sugar and BP levels.

• Recommendation Feature: Suggestions for meals and exercises aligned with user health needs, with an additional capacity to set and track health and fitness goals.

• Monitoring and Tracking Feature: Record and track user's health progress over time, adjusting drug dosages as necessary.

• Medical ToDo list: Reminders for user's scheduled doctor visits and medical tests.
