Create a Service Application Request using WhatsApp automate form

Create a Service Application Request using WhatsApp automate form.

FORM
The form must be able to pick up several important questions.
- Name
- Id number
- Contact Number
- Address
- Post Code
- District
- Number of User
- Monthly average service fees paid
- GPS Location (Using WhatsApp phone share location features).

BACK END (website)
- The form must collect all the data and admin can preview all the entries via web back end.
- The admin can preview/print and download each entry in PDF format.

GOOGLE EARTH
- One of the questions is GPS Location of the applicant.
- When all the report is collected, the system can generate list of GPS locations from ALL applicants.
- The Report must provide an overall view in Google Earth (KML) the locations of ALL the applicants.

APPLICATION NUMBER
- Auto Generate Unique Application Number.
- The system must generate unique running number. The number must start from the 'applicant phone number' + 'running number 001' example: +6017-123-456-789-001

CHECK OUT
- I need the system to request user to pay $1.00 for form submission fees.
- Payment via Visa, Mastercard or Online Banking.

CONFIRMATION
- The user will receive a confirmation email for successful application.
- The user will also receive a payment receipt for the application.
