Create CRUD database with SQLite in Monaca for iOS (Cordova).

I am looking for someone who can create a simple CRUD database (one table - that contains ID (unique), Firstname, Surname) with SQLlite. - no framework (i.e, Onsen, Framework7) but in a simple HMTL5 with both jQuery and Javascript.
