Need Flow in Power Automator to create PDF from Microsoft Form in specific format

I have created a flow in Power Automator. Once a form is submitted via Microsoft Forms, the flow gets the details and emails them as a PDF. This works well, however it displays the fields along the top of the page with "field names" along the top as column headings, and then the data appearing underneath.
I need somebody to assist to change this so that the data appears down the page. See attached file as an example.
