Driver & Dispatcher Dual Form Creation
Budget: $250 – $750 USD
I need two forms, one for drivers and one for dispatchers. The original form will collect information from both parties, and a copy of the original form will be generated. I also will need to generate reports on the information.
Requirements:
- The original form should collect patient details (Name, address of location, facility, mileage, etc...), dispatch details (e.g., location, time, mileage, etc...),
- The second form should be a direct copy of the first, allowing both drivers and dispatchers to input their respective information.
Ideal Skills:
- Proficient in form building software or platforms
- Prior experience in creating dual or linked forms
- Understanding of the transportation industry would be beneficial
