Word Create index referring to paragraph number, not page number

My Word document is pure text—no images.
I've been able to use a concordance file to create index referring to page number, but I need to referring to paragraph number, as discussed here: https://learn.microsoft.com/en-us/answers/questions/4956037/create-index-refering-to-paragraph-number-not-page
I've tested that the StyleRef field in the document is heading 3, but I am not able to add \t {STYLEREF "heading 3" \w} automatically, since there are many index items in my concordance file, manually adding is impossible.
I also have pdf version of the document, it is okay if there is a tool that can do this using the pdf file.
