Styling the Word document (one page) including drawing a specific table and adjusting a header.

I am doing this 'sample loan form' Word document (Coppola Studio), and I would like the header to be stylised similarly to one other company's loan form that it is also attached (Dkt). Body text font on Coppola Studio should remain Merriweather and Open Sans, though. The rest should be very similar.
So, basically, the header should be done properly, and these two tables should be copied. I would need to receive both Word and PDF.
