Psychosexual Forensic Report
Budget: $30 – $250 USD
I have a detailed interview outline that I use when writing psychological reports and I’m tired of copying-and-pasting the same information into different sections. I need that document converted into a single Microsoft Word template where I can type once and have the content flow automatically into the right parts of the final report.
Here’s exactly what I’m after:
• A clean .docx template set up with Word form-field text boxes (content controls) for every prompt I normally complete during the interview.
• Those fields must feed directly into the Executive Summary, which contains the following subsections: Introduction, Key Findings, Recommendations, Psychosexual Risk Profile, and Psychological Testing. When I fill a field in the interview section, the corresponding text should appear in its designated summary subsection without extra steps.
• Layout, headings, fonts, and spacing should remain professional and consistent, so once I finish typing I can export the document to PDF and share it immediately.
Deliverable: the finished, reusable Word template plus a brief set of instructions (one page is fine) showing where to add or edit fields if I ever need to update the structure.
Please make sure all form fields are clearly labelled, locked against accidental formatting changes, and easy to tab through in sequence. Once I can open the file, enter my notes, and see them populate the summary sections automatically, I’ll consider the job complete.
