Psychological Assessment Form Formatting

I need a psychological assessment form to be reformatted to align with specific criteria according to medicaid. It is a biospychosocial assessment that I created, to be completed within 60 minutes; but also gain all the information that medicaid requires for counseling and establishing medical necessity in South Carolina. The edits needed include:
- Adjusting text alignment and fonts
- Modifying margins and spacing
- Improving headers and footers
- Making checkboxes fillable
- Incorporating a drop down menu
