PDF Reformatting for Release Forms

I need release forms for a therapist re-typed and formatted into a fillable PDF. These forms will be emailed to clients, who will fill them in and return via email.

Requirements:
- Medical History: Sections to be filled out
- Consent Statements: Agreement fields
- Symptoms and Conditions: Checkbox options

Ideal Skills and Experience:
- Proficient in PDF editing
- Experience with form creation
- Attention to detail

The current forms are more than 5 pages long.
