Make My Word Document Form

You must be a Microsoft Word Expert in creating fields.

I'm looking to have a simple, basic form created on a Word document that is editable.
I need to be able to edit the entire document, and add as many pages to the document as I want.
I will show the form that I need made to those with expert Word document experience.

Key Requirements:
- The form should be created without any complex elements such as calculations or automated fields. Just standard fields.
- Experience with Microsoft Word and form creation is essential.

You must sign my MNDA before I can share any samples of exactly what I need made.
I will own the copyright to all the images, graphics, files, text, lists, menu's, files, native files.
I will own the copyrights to the work, files, source files, Graphics, and completed files.
You will send me all the full Graphics, work, native files, and source files when completed.
I will release the full milestone when the work is fully completed, checked, and accepted by me.
I do not pay any milestones until the work is fully completed, all files delivered, checked, and accepted by me.
If your work is High Quality and exactly what I need I would be interested in considering you for Future projects.
I don't communicate on Sundays and I don't expect you to work on Sundays.
