Mail Merge QR Code

Hi, I am looking for a dynamic QR code to be added to a word document. The document is used for mail merge. I require, if possible, a QR code to be generated for each individual mail merge, using some of the mail merged fields. This is an urgent project so freelancers offering a quick turnaround will be preferred.
