Editable Word Document Rebuild

I need an existing document transferred into an editable Microsoft Word file. Keep the original layout and text exactly as shown, but delete every element that isn’t core content—specifically the handwritten signatures, any stamps or seals, and the photo. I will supply the logo file in the center above; place it in the same spot it appears in the source so the final file looks polished and ready for reuse.

The finished deliverable must be a clean .docx I can open and edit immediately, with all fonts, spacing, and headings matching the source.
