Desktop Clinical Template Program for Psychologist

I'm a psychologist seeking a desktop application that behaves like a clinical template. This program should assist during patient intakes and assessments.
Key Features:
- Disorder Tagging: I need to be able to tag up to three disorders on the screen during an intake. Once done, it should populate a paragraph stating: "Patient stated that they are experiencing the following mental health issues (insert tags here)."
- Test Scoring: When inputting scores from assessments, it should auto-generate a paragraph based on my pre-set text.
- Scoring System: My scores typically have three responses: good, bad, and variable.
I already have the necessary paragraphs written and ready for input. Please note that I do not require a high-end, complex program as cost is a concern.
Ideal Skills and Experience:
- Strong knowledge in desktop application development
- Previous experience in creating similar clinical or assessment programs is a plus
- Understanding of psychological terminology and concepts will be beneficial
- Ability to deliver a cost-effective, user-friendly program.
The application should support both Windows and macOS. A Graphical User Interface (GUI) is not a necessity, a command-line interface (CLI) could also work, but I have no preference.
