Create docx that looks almost identical to PDF and has bookmarks

This is relatively simple task:

I have a PDF, and I want to recreate it in Word.
Please take a look at the screenshot that is attached to this project.
In the screenshot you can see how I have started to do it myself.
I could do it, but I don't have the time to finish it.
I am looking for somebody who can do it.
You should work accurately, without any "workarounds" .

Here is the exact job description:
You need to create several tables. It should look like the PDF file.
In the PDF you see purple fields. You should type similar names for them in Word and insert a bookmark.
The fields don't have to be purple in Word.

I think it's simple, it just takes like 30-45 minutes.
