- The form will be in greek, we have included some translation in the document to get an idea
- We need to crease 2 online forms, both forms are 100% identical, however to separate them in the database -> table we will create a column with 2 different values

- DB table should be created
- Upon submit all data should be saved into a table and emailed both the user and the admin
- Admin email should include (an application ID (Auto Generated ID), date and time of submission)
- Visual Studio & SQL Express db
