Automated Email Setup from Microsoft Form

I need an automation setup that sends emails from Outlook using information collected from a Microsoft Form.

Requirements:
- Emails should be sent at 1, 3, and 5 months after form submission.
- Email content will be in plain text.
- Use of Outlook as the email service.
- Emails should include form responses.
