IT Managed Service Scope of Work

I am looking for a scope of work for an IT managed service provider

1. Taking handover
2. Supplying supportive hardware and licenses (as listed in BOQ)
3. Provide managed services for the IT environment including:
4. On-site network and voice hardware management & administration
5. Backup and Disaster Recovery

Detailed scope of work including OLA.
