payment report

Good evening everyone. I tried this once before but never could get it to work; so here I am again.
I have a report that I currently use to pay my contractors with. It is set up to pull their data from 1 field called "percentage" and multiply it by the payment amount of the service. "ContPay: [PaymentAmt]*[percentage]"
I have added 2 additional fields to the table "percentage1" and "percentage2". Essentially "percentage" will pay at 100%, "percentage1" will pay at 95%, and "percentage2" will pay 0%. These payments will be based on the number of days that it takes to turn in their invoice notes. There is already a field named "days" which measures this accurately.
I need to be able to set up the new payment method and have no idea how to do this. Can someone take a look and give me some guidance. Thanks!
