Update existing ASP enrolment form

I have two similar ASP enrolment forms. I need to
(1) add a preferred pronoun question with a dropdown menu
(2) add a section on payment for those who choose the paid rather than the free option
(3) make some minor word changes and a minor change in the order of 3 questions.
