United States Postal Service Data -- National change of address Data extraction

The United States Postal Service keeps a record of individuals who request an address change, this is known as the "National Change of Address" (NCOA) database.
A LARGE PART OF THIS PROJECT IS GETTING ACCESS TO THE NCOA DATABASE. I DO NOT HAVE ACCESS TO THIS DATABASE, SO IT IS A REQUIRED COMPONENT FOR YOU TO FIND OUT HOW TO GET REPRODUCABLE ACCESS.
I am interested in individuals who have moved to Nursing homes/Elderly care center/assisted living
The product I would like from this project is
1 -- The list of all elderly care centers/nursing homes/assisted living facilities throughout the entire united states
2 -- A method which would extract the names and previous address(es) of any individuals which may have moved into a nursing home/elderly care center/assisted living.
2.a -- This method should be reproducable and should be able to generate all the required information from individuals within the last X days.
The information should be able to be retreived on an ad-hoc basis.
The final product should be an excel spreadsheet with Columns such as "First name", "Last Name", "Previous Address", "Current Address", "Date Moved", any any other information about the property/individual which could be gathered via your methodology.
On an ongoing basis, I plan on using what you build to extract the names/addresses of people who move into a Nursing home/Elderly care center/assisted living facility, so I should be able to execute this at any given point.
