Simple Database integrated into MS Teams

My team needs to be able to choose between 10 questions to answer. Once they input their answers, those answers must be accessible in one central location. Indicate the question that the answer was related to, whom that particular answer belongs to, and the date and time that answer was inputted. It needs to be accessible through teams or some other Microsoft shareable software. I will need instructions on how to add this to my company system, as you will not be allowed to have access to our network as the creator. This must be done by 12 pm on Saturday, November 26th. I'm open to suggestions on the best software to use. It needs to be simple interaction as possible, as my team is not tech-savvy. I would like it to integrate into teams if possible. As teams are our main way to collaborate.
