Simple Access Registration Form for Club

I'm looking for someone to create a straightforward Microsoft Access or preferably a free database such as BASE registration form for my club. This form will be used to register attendees for a club show, replacing the need for a paper form and then transcribe later. **The ATTENDEES themselves will use 1 or 2 laptops at the door to register.** For a non-profit stamp club willing to pay for a simple database.

The form should collect the following information:
- Name and contact details
- A couple of yes/no questions
- One or two select menu answers
- Simple database for recording attendee information

The yes/no questions will pertain to:
- Previous attendance
- Interest in a volunteer opportunity

For the select menu answers, I need options related to:
- How attendees heard about the club
- Interest in receiving email information for the next year

Ideal skills and experience for the job include:
