Program a form in Microsoft Access

We have a spreadsheet that breaks down personnel certification information by employee, location, certification, etc. Rather than manually filtering and sorting in Excel, we would like to enable users to run a query in Access using an interface like in the image attached. That is, we want them to be able to search by employee ID's (via a textbox) or by any of a set of five parameters (via combo box) and return results. We have a basic database built in Access that imports the Excel data. We are just looking for help with programming the form to ensure that it returns what the user is looking for.
