Microsoft Access developer needed

I need a database created which creates invoices based on clinic room hire for practitioners. I started designing it and have run into difficulty. The database should be able to calculate individual practitioner's monthly room hire, add on additional service charge costs, and take away payment income (such as card payments and vouchers).
