Budget: $30 – $250 USD
There is already an existing excel tool with formulas embedded.
In the excel tool, the new variables to be included are:
Skip variables "Sex- Male"....if male sex is selected, PMTCT status, PMTCT start date will be disabled
Add Phone number column between UAN and Current age
Current age will now have another column for "unit of Time: Years, Months, Weeks"
Add "Pending Result status" and "Pending result date of sample collection" after the 3rd (Last) VL results column.
Disable case sensitive entries or all entries MUST be in UPPERCASE
After the comment column, insert "count down" column: This section will shows
1) Calculated number of days PAST the date eligible for VL
2) Calculated number of days to the next date eligible for VL sample collection
Calculated from the assumption sheet (see attached)
Format VL date and VL result columns for 2nd and 3rd just like "Most current VL and Result
AFTER FIXING THE EXCEL THEN CREATE AN ACCESS DATABASE USING THE FORMULA AND VARIABLES FROM THE EXCEL
