Requirements:
1. To build 2 separate forms that can be imported into my application.
2. One for page 1 and one for page 2 of the attached Word document.
3. The documents must be identical except for the Australian Government logo at the top of page 1.
4. Note: It is unlikely that you will be able to copy and paste from the Word document.
