Convert remote access to mySQL from MS Access to Base database

I have a mySQL database on a web host with an Access (accdb) query and want SQL queries and reports to work on a MacBook. I have the LibreOffice connection. I will provide the accdb and login. Deliverables are odb containing 1. data entry forms, 2. queries, 3. query report. I am open to a different cross-platform solution. See attachments for details, please provide a fixed price or hourly rate+anticipated time required, and anticipated turnaround time. I do not need this immediately.
