Convert MS Access Form to Web page

Using a microsoft access database (one table) as the source for data create a single web page.
Creating the microsoft access web form took me about 30 minutes. I don't have the web coding skills to change it to a website. It's pretty simple and I can provide the Access Database with the data table and the form for you to use as a guide.
This needs to be run on a Cpanel Unix based hosting site - and it must use a Microsoft Access Database (single table) for the data source. when in use I will be uploading and replacing the database once a day.
I need this project done in 24-48 hours.
