IMPORTANT INFORMATION
-I want an Access spreadsheet or program that allows integrated database to the web and offline.
-That patients or people entered into the database can later be modified or information added at visits over 10 visits (2 years of follow-up)
-That all the data can be exported in an Excel file so that it can later be analyzed in a biomedical research program such as R, SPSS or any other that generally read files in Excel 2013
-Access to the database must be protected by one or two simple users with passwords plus another administrator who can modify everything.
-I accept suggestion for better visualization and integration of work
-I add an explanatory Word file with all the variables I want and the form
