Build a audit tool/ database
Budget: £20 – £250 GBP
we want to collect data from patients undergoing botox injections for their migraine headaches. There will be various questions to answer, usually by choosing an option from drop box but some questions with box for free text. The data will be collected on an ongoing basis. Patients usually will come back for injections 3-4 months later and same data will be collected again.
We should be able to see ongoing progress of the individual patient through the database and easily generate graphs and reports for the patient cohort and the individual patient. The database should give us info like how many patients have we treated, how many have improved, how many had adverse reactions and so on. We should be able to use this on an iPad or a computer.
