Auto-populate 2 PDF forms w data from .mdb access DB
Budget: $30 – $250 USD
I have a fillable PDF form that is 2 pages. I also have an .mdb access database that has customer information.
I would like to automate the filling out of the PDF form with relavant data from the DB, and the entire process to be as automated as possible.
There are 23 fields on the PDF that need to be populated with data thats in the mdb database, and some fields are the same on both PDF forms, so the same data will be populated in both PDF forms.
1 PDF form is a customer application for a cash ID card.
The second PDF form is just a template of the cash ID card.
I would like to be able to search or select a customer in the database and it automatically fill the application and cash card ID template with the customers information.
I would like the ability to both capture an image from any attached webcam and that image populate the face picture field on the application and that same face picture should also automatically populate the face image field on the cash card ID template. I also need a browse button to manually attach a fingerprint image we will capture from customer - the fingerprint image will then populate the fingerprint image field on the PDF.
It would be nice if we could have a capture from webcam AND a browse button for face picture as we already have images saved for some customers but for the ones we dont we would like to capture an image on the fly.. and just a seperate browse button just for the fingerprint image.

We would like a standalone software that we can open and it have a searchable field..
We type a customers name, it shows that name from the DB, then we select that name and click a button that says "Create customer CTC documents" and it fill all the fields out (name, DOB, address, city, state, zip, etc) on the 2 pages, then we click a button that says "capture face from webcam" (or browse for saved image) and it populate the 2 face image fields. Then we would like to click a browse button, select the saved fingerprint image and it populate that image field on PDF.
Then if possible, have a button that says "preview and save".. once the preview button is clicked, it should show us the filled out PDF form and give us an option to print and save to desired location.
It can be developed in any programming language.
Attached is an example of the PDF form, image boxes, and ID card template.
Currently we use mailmerge to do this but would like a more automated/direct method.
The fewer clicks it takes, the better.
Does not have to be fancy looking, just fast and functional. Y9u can make the preview either open in default PDF viewer or the preview can be generated inside the software, whichever is easier for you.
I have also attached a drawing of how we imagine the program to look / operate once finished. Please forgive my penmanship ' im not good at drawing. Design/layout is flexible and we welcome your recommendations!
