Access form to log phone interactions
Budget: $30 – $250 USD
As I establish my practice, I'm interested in tracking my interactions with other health professionals. I'd like to log the following in a database form:
Name of practitioner (manual input); date of call (calendar); answered or left message (two options); office location (ordered alphabetically in a drop down...Anaheim Hills, Orange, Villa Park, Yorba Linda, Placentia, La Habra, Fullerton); willing to meet (Y/N); scheduled to meet (date (calendar), time, location (this field would just be a manual input), option to add remarks. I prefer to have drop downs (combo boxes) and have it arranged in exactly as described above.
