Access Data base build VERY similar to the Northwind Template but much simpler

Create an Access Database like quality, design and usability to the standard Northwind Template provided by Microsoft that includes full reporting and queries for Placements, Client Locations, Housing locations, Employees internal and Employees external and lastly Negative outcomes. MS Access Database must be usable in Office 365
There will be 6 tables minimum to include. Travelers, Locations, Placements, Negative outcomes, Recruiters, Pay Advancements. Each Table shall include a form for entry
The databases will include the following information in a fields format.
Travelers:
1. Auto numbered ID
2. First Name
3. Last Name
4. Phone number
5. On Boarding Completed? (“Yes” or “No” drop down)
6. Employable? (“Yes” or “No” drop down)
7. Recruiter (from the recruiter table)
8. Any associated Negative Outcomes from the Negative outcome table
9. Any associate Advancements from the Advancement table
Locations:
1. Auto numbered ID
2. Client Name

4. Client State
5. Client PD (currency)
6. Client Housing (Currency)

6. Should be able to create a Negative outcome from the placement as well and associate the Negative outcome to the record of the placement. A similar effect to as if someone canceled an order.

3. Placement it is associated with from the Placement Table to include the following form the Placement
a. Client Name from associated placement
b. Contract Start Date from associated placement
c. Contract End Date from associated placement

6. Root Cause (Drop Down List) < will use the wizard lookup format to allow for additional entries

Milestone Payments and Releases:
Milestone payments will be used throughout the project. After final approval and acceptance of final payment, you will release all intellectual rights or claims. Acceptance of final payment is agreeing to releasing all rights to work, including intellectual rights, and delivery of product. The delivered database shall be delivered as a template format and compatible with Office 365. For Milestone payments to be released proof of completed work must be supplied via sharing of the database file at its current stage.
