I have a case that requires an in-person visit to a medical facility located at 1 avenue du Professeur Jean Pouhlès, TSA 50032, 31059 Toulouse Cedex 9. During the visit you will need to meet directly with the facility’s admin staff, present the prepared forms I will supply, and gather three sets of information: verification of personnel details, confirmation of specific medical records, and a brief snapshot of the facility’s operational details.

What I need back from you is a scanned copy of each completed form (signed and stamped), a short confirmation note summarising any issues encountered, and two or three date-stamped photos outside the main entrance to prove attendance. All documents must be clear and legible so they can pass internal compliance checks.
