Patient Clinical History Writer

I have a series of patient education materials that now need to be re-shaped into concise, clinically focused histories that other healthcare professionals can review quickly. Your job is to translate the raw information I provide—lab values, imaging summaries, physicians’ notes, medication lists—into clear, well-structured patient clinical histories written in accurate medical terminology yet easy to scan during rounds or hand-offs.
What matters to me is precision, logical flow, and consistency with standard clinical documentation style (SOAP‐like narrative works well). Each finished history must:
• begin with a one-sentence presenting complaint,
• follow with succinct history of present illness and relevant past history,
• list active problems and current medications,
• close with key investigations and the current management plan.
I will supply anonymised source notes in Word or PDF; you return polished histories in the same format, ready to drop into our EHR or print as a hand-out. If you are used to AMA style and comfortable referencing ICD-10 codes where appropriate, that’s a plus, but the core goal is clarity for busy clinicians. Turnaround is flexible yet prompt communication is essential so we can iterate quickly on tone and depth.
